I use an app called AntiRSI.app to manage my computer breaks. It makes you take micro pauses (look away from the monitor, don't touch the keyboard or the mouse) a couple of times every hour, and a full-on work break every hour (50 minutes is the default I think, I've changed it - you can customise AntiRSI.)
I find it helps me remember to stretch and take time away from the nerdbox. Every micro-pause I make sure I focus on something in the distance, and every work break I make sure I get up and walk somewhere.
